New Patient Registration

If you currently reside within our Practice Boundary you can begin your registration as a 'New Patient' by completing the Registration form.

Important: Once you have completed the online form you will need to come into the surgery to complete the NHS Family Doctor Services Registration (GMS1) form and provide both photographic ID and proof of address dated within 3 months to complete your registration. Please then allow 3 working days for us to process your registration.

To register a child under 16 years of age with the practice please complete our Child New Patient Registration form (under 16).

Registration Forms

When filling in the registration form it is important that the form is fully completed to comply with the Health Authority requirements:

  • Previous surname (if any)
  • Town and country of birth
  • Home address with postcode
  • Telephone number
  • Previous address
  • Name and address of previous GP
  • If appropriate, date of arrival in the UK

Once you have completed the online form and visited the surgery to complete the Family Doctor Services form (GMS1) and provided the required identification we will aim to complete your registration within 3 working days.

Temporary Services

If you are ill while away from home or if you are not registered with a Doctor but need to see one, you can receive emergency treatment from the local GP Practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local Practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that Practice.

To register as a temporary patient simply contact the practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a Practice in the town or area where you are already registered.

Your Named GP

Stanhope Surgery have allocated a Named Accountable GP for all of our registered patients. New patients joining us will be advised of their Accountable GP at the point of registration. If you do not know who your named GP is, please ask a member of our reception team.

Your named GP is for care coordination purposes only and therefore you are eligible to see any GP within the practice.
